คำถามทางยา
ยา cisplatin เป็น platinum agent ใช้รักษาโรคมะเร็งหลายชนิด เช่น มะเร็งอัณฑะ (testicular cancer), มะเร็งศีรษะและลำคอ (head and neck) มะเร็งปอด (lung cancer), มะเร็งทางสูตินรีเวช (gynecologic cancers) และอื่นๆ โดยการใช้ยาจะทำให้เกิดผลข้างเคียง เช่น คลื่นไส้ อาเจียน ประสาทหูเสื่อม (ototoxicity), อิเล็กโตรไลต์ในร่างกายไม่สมดุล (electrolyte abnormalities) และมีพิษต่อไตได้ (nephrotoxicity) ซึ่งในผู้ที่ใช้ single dose of cisplatin (50 mg/m2) ประมาณ 28-36% มีโอกาสเกิด nephrotoxicity
ยา mannitol เป็นยาขับปัสสาวะกลุ่ม osmotic diuretic ใช้เพื่อลดการเกิด cisplatin-induced nephrotoxicity โดยไม่ทำให้เกิดพิษและลด half life ของการกำจัดยาที่ไตได้ (1) การศึกษา meta analysis พบว่าการใช้ mannitol มีประสิทธิภาพและความปลอดภัยในการลด cisplatin-induced nephrotoxicity events โดยสามารถลดอุบัติการณ์การเกิด renal injury events โดยเฉพาะอย่างยิ่งใน grade 3 kidney injury events โดยไม่ได้เพิ่มความเสี่ยงต่อการเกิด hypomagnesemia, hypokalemia และ hyponatremia แต่การใช้ยาจะทำให้เกิดอาการคลื่นไส้อาเจียนเพิ่มขึ้นได้ (2) โดยสารน้ำที่สามารถผสมกับ mannitol คือ Water for Injections ซึ่งมีความคงตัวที่ pH 4.5-7 สำหรับสารน้ำที่ไม่ควรผสมกับ mannitol คือ Potassium หรือ Sodium chloride เนื่องจากจะเกิด Precipitation ได้
การบริหารยา mannitol จะให้แบบ intravenous infusion เท่านั้น ซึ่ง total dosage, concentration และ rate of administration จะขึ้นอยู่กับโรคที่รักษา ปริมาณสารน้ำที่ควรได้ และ urinary output โดยขนาดทั่วไปในผู้ใหญ่คือ 20-100 g in 24 hr แต่ในกรณีต้องการให้มีการตอบสนองอย่างเพียงพออาจให้ 50-100 g in 24 hr โดย rate of administration คือ 30 - 50mL/hour. สำหรับสารน้ำที่ไม่ควรให้ร่วมกับ mannitol ได้แก่ เลือด ซึ่งหากให้ร่วมกันควรให้ 20mEq of sodium chloride ต่อ 1 ลิตรของ mannitol เพื่อป้องกันการเกิด pseudoagglutination และการให้ potassium หรือ sodium chloride ร่วมกับ mannitol อาจทำให้เกิด precipitation ได้ อีกทั้งมีรายงานว่ายาปฏิชีวนะบางตัว เช่น cefepime, imipenem หรือ cilastatin อาจ incompatible กับ mannitol
สรุป ไม่ควรให้ mannitol ร่วมกับ DNSS 1000ml + KCL 20 mEq +50%MgSo4 เนื่องจากมีส่วนผสมของ sodium chloride และ potassium chloride เมื่อผสมกันอาจทำให้เกิด Precipitation ได้ ดังนั้นอาจให้ mannitol ก่อนหรือหลังให้ cisplatin สารน้ำและอิเล็กโตรไลต์ไปแล้ว
