ยา Zoledronic acid ขนาดที่มีในรพ.คือ Zoledronic acid Inj. 5 mg/100 ml และ 4 mg/5 ml เป็นยา nitrogen-containing ในกลุ่มยา third-generation bisphosphonate
มีข้อบ่งใช้รักษาโรคกระดูกพรุนในหญิงหลังหมดประจำเดือน โดยมีฤทธิ์ป้องกันการสลายกระดูก (antiresorptive agent) จากกลไกที่จับกับ hydroxyapatite crystals ที่กระดูก(bone mineral matrix)
และออกฤทธิ์ยับยั้ง osteoclast activity การบริหารยา 5 mg IV infusion โดยสารน้ำที่ผสมได้คือ Water for injections และ infusion time ควรมากกว่า 15 นาที (1)
มีประสิทธิภาพช่วยเพิ่มความหนาแน่นของกระดูกส่วนเอว (lumbar spine) และคอกระดูกต้นขาบริเวณสะโพก (femoral neck) 6.7%, 5.1% ตามลำดับ
และลดอุบัติการณ์การเกิดกระดูกสันหลังและกระดูกสะโพกหักได้ถึง 70% และ 41% ในหญิงหลังหมดประจำเดือนที่เป็นโรคกระดูกพรุน (2) ซึ่งการให้ยาแบบ intravenous infusion
จะให้ไม่ต่ำกว่า 15 นาที และ Flush ด้วย sodium chloride 0.9% หลังจาก infusionแล้ว โดยก่อนให้ยาแนะนำ prophylactic ด้วย Paracetamol IV 1g 30 นาที (3) หรือ หลังจากให้ยา
zoledronic acid infusion แล้ว 4 ชั่วโมง แนะนำ ให้Paracetamol 1g or ibuprofen 400 mg q 6 hours เป็นเวลา 3 วัน เพื่อลดอุบัติการณ์การเกิดอาการ transient postdose influenza-like symptoms (4)
สรุป ควรให้ premedication ด้วย paracetamol 1 g 30 min หรือให้ Paracetamol 1g or ibuprofen 400 mg q 6 hours
หลังจาก zoledronic acid infusion แล้ว 4 ชั่วโมง เป็นเวลา 3 วัน เพื่อลดอุบัติการณ์การเกิดอาการ transient postdose influenza-like symptoms
